A Psiquiatria de Ligação na abordagem das Síndromes Funcionais Gastrointestinais – um estudo no Centro Hospitalar de Vila Nova de Gaia/Espinho

Resumo


Introdução: as Síndromes Funcionais Gastrointestinais (SFG) representam uma patologia prevalente, mas que ainda colocam dificuldades de diagnóstico e terapêutica, com impacto individual e social importantes. Por isso, pretendemos averiguar a abordagem atual dos doentes com SFG num hospital geral distrital português. 

Material e Métodos: analisámos descritivamente os pedidos de colaboração psiquiátrica feitos pelos médicos gastrenterologistas ao Serviço de Psiquiatria do Centro Hospitalar Vila Nova de Gaia /Espinho no período de doze meses. 

Resultados: foram referenciados à Psiquiatria 21 doentes, no espaço de 12 meses, com suspeita de patologia comórbida gastrointestinal e psiquiátrica. O objetivo consistiu maioritariamente no esclarecimento de diagnóstico de SFG em doentes com diversas queixas gastrointestinais, sobretudo diarreia (25%), dispepsia (25%) e dor abdominal (25%), mesmo sem queixas psiquiátricas aparentes em 23,8%. 

Discussão: identificámos algumas lacunas na atual abordagem dos doentes com SFG neste meio hospitalar, que se prendem com a falta de diretrizes na colaboração entre Psiquiatria e Gastroenterologia para o diagnóstico e acompanhamento dos mesmos. Dessa forma, o seguimento conjunto ficou reduzido a 42,8% dos casos. A intervenção multidisciplinar nas SFG já demonstrou benefícios e poderia solucionar este problema.  

Conclusões: é pertinente a criação de um modelo multidisciplinar de abordagem entre a Psiquiatria de Ligação e a Gastroenterologia para garantir a resposta adequada às a necessidades dos doentes com SFG.
